By submitting this registration for my child, I acknowledge that I am being asked to attend a required orientation meeting on March 10 at 6:30 p.m. where I will be asked to sign a permission form for my child. I further understand that if I do not attend this orientation or make some other arrangement with UUFD Coordinator of Faith Formation (faithformation@durangouu.org), that my child may not attend OWL classes.

All Others, $100.
