Youth / Child Enrollment for Faith Formation

Once a year (or as new children come into Faith Formation classes), to best meet need the needs of families and children, we ask for a "enrollment" form to be completed for children participating in faith formation activities at the Unitarian Universalist Fellowship of Durango. This information helps us to plan more effective, provide the most engaging content possible, and create a rich experience for participating children. Since we have not gathered this information since before the beginning of the covid pandemic in March of 2020, this will also help us ensure our records are up to date. Child Safety Procedures

All childen and youth under 18 years of age must be supervised by an adult at all times when on the property of Unitarian Universalist Fellowship of Durango, including virtual spaces.  Children participating in faith formation activities who are in the 5th grade or younger must be picked up by a family-approved adult.  Youth in 6th grade and older will be released from faith formation activities without being picked up by an adult.  

Events beyond the typical Sunday faith formation activities where children attend without a parent present will be required to fill out a permission form.  This includes youth who are of driving age but have not yet reached the age of 18.

Photo and Video Use

Throughout the year, there will be opportunities for recorded images to be taken of children and youth.  We sometimes use these images in the UUFD newsletter, News from Faith Formation, and on the UUFD website (durangouu.org).  Please take note of the various options.  If you wish to opt out of all, mark only the final item in the list.
